Reliable discounted membership sales and failed-setup cleanup

Staff can again sell a membership with an agreed recurring price, registration fee waiver, referral offer, or other coupon-backed concession. Membership creation now uses Stripe's current subscription-discount request shape while preserving the existing concession precedence and retry fingerprint.

**Clean failed setup** can also remove a processor-rejected membership attempt
created by the crash-safe subscription workflow. Booking Bible retires only the
exact unbound technical request for that failed attempt, then repeats the full
tenant, processor, lifecycle, payment, booking, contract, entitlement, ledger,
and foreign-key safety checks before removing the pass. Any refusal rolls the
technical cleanup back, so real or uncertain membership history remains intact.
